How is the application process for assistive device?

An application is sent electronically through Gagnagátt or Saga medical record by healthcare worker.

The application must include:

Justification of the need for the assistive device on skills, disability and health
Information from the Medical Records on diagnosis (ICD Number) and Medical History in the case of first application
