Attachments must be original documents in PDF format, photocopied or scanned in colour, and high resolution to display all the information on the original document.

  • Scanning/photocopying must be perpendicular, as optical reading processes information. Furthermore, scanning/photocopying must cover all visible content on pages, including if something is written on the edge of the page.

  • All pages must accompany each document, even if only part of them is referenced.

  • Pages must be marked with a serial number. If original documents do not have page numbers, they must be labelled with a serial number and ensure they are in the correct order.

  • Copies of medical records must contain all forms, notes, research results, and entries, including handwritten notes, certificates, pictures, etc. All health records of the patient's discharge/treatment/surgery must be included. It shall be noted if something is left out and then why.

  • If it is impossible to comply with the above or if data is omitted, this should be stated with explanations.

  • Unclear/incomplete copies of documents will be rejected unless the sender justifies that better copies cannot be provided.
