Follow-up after hysterectomy

Women who undergo hysterectomy where the indication for the procedure is premalignant cancer or where the post-operative diagnosis is precancerous changes in the cervix are at increased risk of vaginal intraepithelial neoplasia, VaIN, and invasive vaginal disease.
The follow-up should be based on clinical assessment but the following is a guideline:
Post-hysterectomy tissue analysis shows that precancerous changes are completely removed, surgical edges clean
Vaginal sample for HPV - 6 months after surgery
hr-HPV negative
No further follow-up
h-HPV positive
Referral to colposcopy
If no signs of VaIN, follow-up is stopped.

CIN 1 tissue analysis
Vaginal sample for HPV 6, 12, and 24 months after surgery
h-HPV positive
Referral to colposcopy
Hr-HPV negative
Follow-up is stopped after 24 months
CIN 2-3 tissue analysis
Vaginal sample for HPV 6, 12 months after surgery
Vaginal sample for HPV every year for the next 9 years
Follow-up should be until the age of 65 or at least 10 years after the surgery
